Idea:
Get one of cheap 532 pointers/modules and take out the KTP and the IR filter and you got 1064
Granted, it might still leak some 808... but if that's not a problem, then any pointer from www.dealextreme.com would work

I have a q-switched 1064 system (guts from a laser-export 355nm laser) but it's probably way too much power for what you want
I should chime in--said 1064nm module out of the laser export will run CW just fine, I measured it doing a few hundred mw (can't remember how much exactly off the top of my head). Just apply power to the diodes/tec, leave the q-switch disconnected and it will lase CW quite happily.
