are you sure the voltage and amps of the power supply feeding the flexmod are appropriate? (not too low?)
are you sure that the v to ui is soldered correctly?
then are you sure that it is the v and m sorted together and not something else?
have you unsorted the laser diode output?

sorry for asking simple questions but it is something simple you are missing now and i'm too sleepy rite now to think of anything else...

hope it all works out for you or someone in your own timezone catches up to this thread

The power supply is 12v at 2 amps
V+ and UI are soldered together through a wire on the bottom of the board.
I am sure the V+ and M are shorted together.
What do you mean have you unsorted the laser diode output? would pics help?

i just mean that at some point the manual says to sort the laser diode output ( + and - )

this is supposed to be just for a few seconds. so make sure they are not in contact with each other, or that your DMM is not connected to the flexmod on power up

that's all i can think of at the moment

"After 7 seconds and if the interlock remains closed, the driver will
go into operation mode and accept modulation input control. If the interlock circuit is open initially, or if the output is open
circuit, a red LED will illuminate and no current will be passed to the laser diode until the diode circuit is complete and the
    interlock reset.
If, at any time during operation the output circuit goes open, the driver will shut off output and illuminate the red LED until
the output circuit is closed and the interlock is reset."

No it is not a good idea to set when the diode's connected. You should use a dummy load and a meter.

Have a look at some specific threads relating to the FlexMod 3 and see if the answer is there. The issue you're facing sounds similar to others. Dr Lava has responded to those queries, he would likely respond to yours if you post your question in those threads.

One thing to try is setting the gain to a low setting so that the demands of the driver don't exceed your supply (this driver can be powered up to 24v and supply up to 4A. So if your gain was too high, your PSU would not be powerful enough.

grab a DMM and check the actual voltage of the wallwort - this one in particular might have a much higher OCV and may be messing with the driver.
