Annual CDRH Reporting

As the deadline is fast approaching to file the annual show report for those of us in the U.S., I just want to make sure I am using the correct form. I belive it's form 3636. Do you send in all 17 pages, including the ones that are "deliberately blank" or, just 4 pages that comprise the report? I'm guessing just the 4 pages but, since this is the first year for me to do this, I want to be certain I'm doing it correctly and with the correct form.

Edit.... Never mind the question about sending all 17. I read it thoroughly and it's only the four and, in my case, since I'm not claiming to have done any "shows" in the short time I've had my variance, it looks like I only need to submit pages 1 and 4.
