Variance questions

Hello
Ive been searching here for hours and still not a 100% sure what I need to do. I'm in the process of building a projector. So Ive found 3 forms on here that people talk about, the FDA 3147, 3632 and 3640. I assume you fill out the 3147 for each show you do, but which form do I fill out for my projector?

Step 2- Variance application for the *PRODUCT* **NOT** a show!! http://www.fda.gov/downloads/aboutfd.../ucm080788.pdf

Step 3- Laser light show report. http://www.fda.gov/downloads/AboutFD.../UCM081634.pdf

Step 4- Laser light show Variance. (Same link as product variance link). This variance application is for a *SHOW.*
