Does anyone have the new number for faxing "Laser show notifications?"

    Is this even required anymore??

    Connecticut.

i now have 3 different numbers that supposedly are for this purpose. 1 is a library number. 1 is 24/7 busy signal. and 1 NEVER has a fax machine pick up. it just rings and rings and rings and rings. (NOT even a voice mail!)

I'll try to hunt down what I have on Connecticut. Check your state phone directory to see if you have a radiological bureau. They would be the ones who may or may not require it-

How about posting it so we can reference it in the future?

Hey Cats - Ok, here is most recent version of 'the Bible': (uh, US-version, that is!

A pretty-good (not always up-to-date, but about 80-90% there - gets you in the right direction anyway) resource for State Regs and Contacts http://www.rli.com/resources/state-contacts.asp - (ie; CT requires "Written notification as required by FDA" , which more or less is just a 'courtesy notice' - nothing hardcore like NY or TX reqs...

And most-current address for all Show/Projector/Annual Reports for CDRH should go to:

Center for Devices & Radiological Health
    Director, Office of Compliance (HFZ-342)
    9200 Corporate Blvd.
    Rockville, MD 20850
    FAX 240-276-3272

Most current info they have on the web is here:
    http://www.fda.gov/cdrh/radhealth/products/lasers.html

And FAA's most current regs/calcs/info start here: (4+ pages...)
    http://www.faa.gov/airports_airtraff...R/air2901.html

    Regional Office addresses are here:
    http://www.faa.gov/about/office_org/...ions/regional/

Also, here is some good general safety articles from ILDA (though some of it is dated, but still fairly-relevant..): http://www.laserist.org/Laserist/Safety.html

PM-me if you ever need any help on Variance prep/FAA prep/Regional Contacts info... if we get Greenaliens' excellent suggestion of a 'Laser Safety' official forum opened, I will be glad to post all we've ever learned about Variance prep, working with FAA, etc - handed down to us from an 'old Jedi master' years ago, and we are happy to 'share the love'! -

    ciao

This is all fantastic information to have quick access too. Can this be "pinned" at the top of this forum so it doesn't get lost among all the other threads?
