just thought i would post some pretty important forms for the FDA/CDRH.

i know when i first started out, trying to find the forms and info needed was next to impossible.

i tried to gather the most useful and needed forms in one package for you to download. (if you so desire.

)

if anyone can think of any important forms or notices i missed, feel free to let me know and ill update the package.

Sorry to grave-dig, but I just noticed that the link to Marc's forms is not working anymore.

Marc, when you get a chance, how about uploading the archive to the FTP server?

Hi Adam,

There is a zip file in the /Documentation directory on the FTP server that contains the same forms Marc provided. I discovered this after I had uploaded my own copy of the forms!

Nope - that's a different Kyle. It seems he goes by "Laseronics" here on PhotonLexicon. I'm just a casual laser enthusiast slowly saving up funds to build my own projector.
