If cut "by hand" (as opposed to machined that way) its more likely he used a bi-metal hole saw kit and a metal punch (for aligning the pilot drill).

Cutting an accurate circle manually is nigh on impossible. I've heard of people doing it on computer cases with a Dremmel, but I'd still like to see it done as having tried cutting circles with a Dremmel its impossible for mere mortals!!
